use of representative question

i have no representative so i have not checked it in checklist and will not attach it with my application to CIO.
please confirm if this is the way to do or we need to provide all the options of checklist.

Perfectly alright. Only thing is -put a X on that & write, "Not Applicable" -if u want to.

where do we need to mention about our old application, as i have withdrawn my old application and want to mention it.
is it in schedule 1 (point number 9 and check box 2 and then mention reason there )
