Use of Representative Form

HI guys, need some help filling out Use of A Representative Form.

Question 3
If you have already sumitted your application:
Name of the office where the application was sumitted.....

My question is:

Should i answer this question even if this is the first time my application is gonna be submitted?

Please advice as soon as possible my forms are ready to leave.

Are you using a representative? This form needs to be filled only if you are using a consultant

If yes, then your consultant should be helping you if no you do not need to fill in this form

Any ways, do not leave any field blank say N/A if it is your first application

Yes, i am using a representative but it is a friend from the company that offered me a job, the company wanna be able to call CIC and find out how the process is going on.
My first application was return but i never got it, so they decided to deal with CIC also.
Tha is why i am not sure if i should answer that question or not.

You can use additional sheet if space is a problem
