I am in USA. I did my medical exam today. The DMP said he will send my IMM1017 to CIC with my photo on it. He told me again and again to save the copy of IMM1017 (green) which he gave it to me and Don't send it with my application. But IMM5491E #27 says I should send the original copy 2 of IMM1017. What should I do?
The IMM1017 which DMP gave me (green) doesn't have my photo on it. There is a maple leaf stamp with the number on it, half on the form and half on the place where the photo should be. The DMP did sign his name on it. But he didn't ask me to sign. Is that right?

You should keep the original of the IMM1017 - but you are required to submit proof that you underwent medical examination with your initial submission of the application. So send a photocopy of IMM1017 with the application.

The DMP is probably confused because applicants immigrating in merit based classes need to wait for CIC to request proof of medical examination - they don't send it with the initial application. However, applicants being sponsored are specifically instructed to undergo examination before submitting the ap - and to include that proof when they submit their application.

And, no, I don't remember having to sign it.
 
We sent the original because that is what it stated on the document checklist. I'd copy it, but send the original.

We did send an email to CIC and are waiting for their answer.
 
No problem.

It stated very clearly that we were to send the original...I think that is true for all regions...so if I were you, I'd send the original.
