Schedule A asks for details prior to turning 18 and asks for your name in your native script in case they need to use it to verify details in your home country.

CIC usually asks people with refusals to submit schedule A. I submitted it in advance so they don't waste time requesting it later. You can google it and you can find the form online.
I did not get schedule A requested even though i was ordered to leave US, got my COPR and Passports last week. So it really depends on the officer and VO.

So getting us visa refusal doesn't affect our canada application?

Some are asked while others are not. I just didnt want to wait to be asked so I submitted it anyway.
