We recieved our CSQ from Quebec June 2011, and we immiedately filed for federal PR application June 2011. The Immigration Canada sent out the requrest for medical examination in Nov 2011, but somehow we didn't get it either in mail or by email. We moved, but we send the change of address request, which wasn't recorded by Immigration Canada. The email was missed out by my husband!! :o Today my husband found out the old email. We are trying to find out if we can use the old forms sent out by the Immigration Canada back then for medical examination, or we should wait for the Immigration Canada for new forms to proceed. We contacted the Immigration Canada by email and are waiting for their reply. Anyone has any suggestion about how to move forward from here? We live in Quebec with valid work permits, so I am not so concerned about our status. I feel so silly that we have waited for so long and the most important email was sitting there forever to be discovered!

I guess is that we should proceed to do medical examination as soon as possible, and at the same time explain to the Immigration Canada why it was delayed...

I do not see any due date on my medical forms and in my email, so I think that you should proceed immediately whit your medicals as you said and also inform the VO about the change of address.

If the experience of other members in similar situations is anything to go by, you are not likely to get a reply. So you might as well use the section A forms you received and proceed with the medicals. You can also inform the visa office by email that you've done it and attach a copy of the medical receipt.
