Hi All

I just noticed that after you fill out the form and then take a print out,the column on the top right hand corner of the first page for "OFFICIAL USE " and the page numbers found on the bottom right hand corner of each page disappear. These two only appear when you click on"PRINT BLANK FORM",but then the filled out information is missing.

How have you guys handled this?

Please advise.

Hi, I noticed the same thing, the "Official Use" box doesnt appear unless you "Print Blank Form". This is the most stupid form that I have ever seen. Totally useless.
I printed it as blank, and wrote on it with a blue pen, make sure to write in clear words, write slowly. And make sure that you put N/A wherever you have a blank field, or wherever that criteria doesnt apply to your case.
I submitted my application a couple of days ago.
