URGENT: PMM or other Immigration Experts - Please advice!

I applied for Canadian immigration last month and the whole package was returned back by the Buffalo Canadian Consulate because I did not put N/A for several sections in Appendix D.for example...
1. Membership with Organisation
2. Millitary Service etc...

The application form has a date stamp of Consulate in the photograph column.

My question is should I just fill out the empty sections in the same form they returned and send it back or should I create a new one from scratch.

Can I fill out the empty sections with a Pen? I typed on the original one I send to CIC.

Just correct the form and re-send it.

Should I add a cover letter explaining what sections I filled out?

Can I make the changes with a pen? I just dont want to create a bad impression.

As long as it is legible it is fine. No you don't require a cover letter.
