#1:The employer for whom I worked 6 years back is no more. What happens if CIC does a background check and no one is there to confirm my employment?
#2:After I validated the generic application form, I got 2 bar code pages 5 & 6. But the 6th page has both the barcode and my name and signature and the 5th page is full of barcodes. Is this normal?, If yes, should I place both pagee 5&6 on the top of the application?
#3: where should the docs checklist be placed? at the end of the app forms?
#4: where should the payment form be places?at the end of all the supporting docs in the order of the checklist?

You can still use someone as a reference from your old employer (i.e. past manager, co-worker, etc.). You can just include a note, or mention it in your reference letter, when the company went out of existence.

Evidence of your employment (T4s, Pay Stubs, Contracts, etc.) will help demonstrate that the employer previously existed.
 
Even less of an issue - your qualification in this class is based on your Canadian experience.

#5: Where should I place some additional docs related to my employment outside Canada?
 
Have some patience. No one is being paid to reply here, so give your post a reasonable amount of time before bumping (15 minutes is not reasonable).

From the application guide:

Place the barcode pages on the top of your application (forms and supporting documents) when you will submit it.

Therefore place both at the top.

It is not mandated - many people put it after the barcode pages but it won't make a difference.
#4: where should the payment form be places?at the end of all the supporting docs in the order of the checklist?
Same as above, doesn't matter. The only specific guidance they provide is for the barcode pages to be at the top, and not to bind your documents (don't staple, use paperclips).

I would suggest putting everything else in the order of the checklist to make it consistent. But the intake office takes your application and sorts it by their system (to assess if your application is complete), so it really doesn't matter.

The checklist says that it has to be included as the cover page for the application but the instruction guide says that the bar code page should be at the top. I am confused. Please help.

Not a big deal - place bar code page at the top and the checklist immediately after like so: 1.) bar code page, 2.) checklist. 3.) place documents in order of checklist.
