URGENT NEED-FILING THE FORM FOR ADDITIONAL FAMILY...PLZZZZZZZZ

I am the Principal applicant and my spouce wants to fill her portion of the forms especially the Additional Family.

Question
1. Under section A where would she fill? Would she fiil as applicant and fill my name as spouse?

2. Would she fill my name as applicant and fill her name as spouse?

3.Please note that I have already filled my portion as applicant and included her as spouse.

Please seniors i need urgent reply.
