Urgent Help for adding child

I need to add child into my existing file. CIC ask me to answer some questions on couple of forms.

My question is that Do I need to answer all question again or just specifically relate to dependent?

Do I need to send full forms with signed page or just the one they asked along with signed page?

I am sending thru email.
