An affidavit is enough.
There is a standard format for " One and same name". Use that and get notarised.. . No need to reject ITA
Am not sure if sending a case specific inquiry would help in this case. But you can try to do so and attatch the affidavit.
If it was possible to send a request to change the name on the application to match that on your passport it would have been more helpful.
An affidavit will only show that the names belong to the same person but it wont change the fact that your passport (which is the main identity document used in your application) has a different name.
Another option would be to change the name in your passport to match (depending on how fast your country processes passports).
If you decide to apply afresh or go for the ITA, try to put your name as itnappears on the passport, and in the section where they ask if you have ever used another name you can writenyour name as it appears on your educati
