URGENT !!! Declaration as to the Truth of Nominee Application

I need to fill the application form for FSW pilot ontario but they require an affidavit on the last page that what information i declare are true correct and complete. I am staying in kuwait and can anyone tell where can i get this done .. I dun have much time and the last day for acceptance of application is 4th May 2012. Senior members please help
