URGENT!!!

I am filling my forms for express entry, and I faced a problem about my personal history, I had worked 4 different places as a mechanical engineer and earlier as technician and I just claimed point for work experience in one company which I had reference letter from (so, I don’t have to provide reference letters from the other 3). But my problem is that I mentioned my work experience in my study permit forms and there were some mistakes (which was made by visa consultant). My job title were inserted different from what my actual job titles were (I was a mechanical engineer and technician, but my job title were all mentioned as site manager assistant, which was part of my duties but not my job title). Moreover, one of my work place’s company name was wrong (it’s actually the name of company that use to own the company I worked for it)
Later on in visa extension forms, I tried to fix it by putting my actual job title and put “site manager” in parentheses to fix this issue a bit. And, right now, I want to use “mechanical engineer” as my job title.
I want to double down that I just use one of my work experience for work history, and I am providing reference letter for it as mechanical engineer to get points for my work experience and I don’t have reference letter from the other places.
This makes me really worried. Is it considered as misrepresenting? What can I do?
I want to know:
1- Do I have to put my job title as “mechanical engineer” or “mechanical engineer (project manager assistant)”?
2- Do I have to put the company wrong name (actually the old name) as the former owner of the company in parentheses or just mention the new name?
