Urgent:BOWP Questions

Hi,

I am applying BOWP for my self and extension of status documents for Family.

Below is list of supporting documents and my question
1. Passport (required) - Clear
2. Use of a Family Member Representative for Online Applications (IMM5713) - Clear
3. Marriage License/Certificate (required) - Clear
4. Digital photo (required) - Clear
5. Proof of medical exam (required) - I got medical done for PR last month, I believe same report will be valid here
6. Family Member Proof of Status (required) - What should I upload here, my whole family is in Canada

Also I do not see digital photo upload section to upload digital photo of my wife and child
