CIC sent us a letter saying this: Please submit an updated version of Schedule A Background /

Declaration form (IMM 5669) for you.

In section 8, specify in English or French the months and years of each of

your activities and indicate your precise occupation (exact job title,

unemployed, student, visitor, detainee, etc.) since you are 18 of age.

Please avoid unclear information such as “employed” and leave no gap

between activities.

In section 12, specify in English or French the months and years of each

of your addresses where you lived since you are 18 of age. Leave no gap

between addresses.

The following gaps need to be covered:

For your Schedule A form:

In section 8 Personal History, from 11/2014 to date.

In section 12 Addresses, from 11/2014 to date.

This document has to be duly completed as you will not have another

opportunity to do so. If you fail to follow these instructions, your

application may be refused.


Do we need need to re-do the form, or just fill in sections 8 and 12? Thanks
 
You did save backup copies of the forms right? Just fill in the gaps they asked for. If you didn't save copies, you should fill it out again with the same information but fill in those gaps.
