Unable to open IMM5645 family information form

Hello,
Am trying to open and download family information IMM 5645 form but it says
“The document you are trying to load requires Adobe Reader 8 or higher. You may not have the Adobe reader installed or your viewing environment may not be properly configured to use adobe reader.
For information on how to install Adobe reader and configure your viewing environment please see https://adobe.com/go/pdf_forms_configure”
I have adobe reader installed and all other documents are opening in pdf except this form. Please can someone help me on how to download this form and open.

Try opening this in internet explorer. Works fine.
