I have a valid Canadian work permit which I have not used at all. Now I want to travel to canada (for short duration)

along with my family.

While filing up the canadian visitor visa eligibility questionnaire, it asks for 'visa fees waiver for self' do I answer this question as yes

and proceed or how?

Moreover do I need to fill the visitor visa form IMM5257E for self even though I posses a work permit?
 
It sounds like you're from a country that requires visas to travel to Canada. Having a valid work permit won't allow you to enter Canada, it only allows you to work here. So yes, you need a visit visa (Temporary Resident Visa) as well.
