I need all you comments for document list section "Travel document or Passport"

Whether we need to provide only Passport or Passport and Visa?

Note : I have Canadan Visa, workpermit and US B1 Visa.

Only your passport is needed. Provide a photocopy of the photo page and all pages containing such information as date of birth, changes in names, etc. In most cases all the necessary information is printed on the photo page so it is sufficient. No visa pages needed.
