TO and FROM field

Lets say I have been living in a house for the last 10 years so in the TO field I would write 1994-09-30 and in FROM field I cant write PRESENT and it has to be a date. So because I am submitting application now I would write 2014-09-30 even though I will continue to live here. Is this correct?

From is the date you moved in and TO is the day today.
