Therapy Fellowship

Hello,
I have applied for a Hand Therapy Fellowship at Roth McFarlane Hand and Upper Limb Centre at St. Joseph's Health Care, London, ON, Canada (https://www.sjhc.london.on.ca/hulc/hand-therapy-fellowship-program).
The fellowship is unpaid (no stipend or salary) for a duration of 4-months offered by Roth McFarlane Hand and Upper Limb Centre and is not affiliated through any university or institute. I will be learning and treating patients as a part of fellowship program, will obtain necessary license and insurance required by the fellowship program. This fellowship is like an educational program with a $1500 CAD fee.
I am an Indian Citizen with H1B visa currently living and working in the United States of America. I don't have a green card and I am not a permanent resident of US.
Please guide which type of visa shall I apply for to be able to pursue this Hand Therapy Fellowship at Roth McFarlane Hand and Upper Limb Centre at St. Joseph's Health Care, London, ON, Canada (https://www.sjhc.london.on.ca/hulc/hand-therapy-fellowship-program).

It sounds like a training program that is less than six months in length - so you would apply for a TRV (tourist visa) to attend this program.
