Hi,

I am in the process of applying for canadian citizenship, and would like to inquire about the supporting documents that might be included/requested down the road:

1) Passports: What do you need to translate in a passport? Do you translate any page that does not have an english or french version? Do you translate the entry/exit stamps only (if not in english/french)? Do you translate visas?

2) OHIP records: I am assuming these are records listing what OHIP had paid for my medical services in Ontario, and they prove that I was living here, but where do you get them from? Do they have a 'formal' name?

3) Would you send originals or photocopies or certified copies? I am talking about documents like bank statements, paystubs,...and the likes.

- Call OHIP and find out the procedure.
- Documents also include employment letters, tax related and other bills, etc.
- Bank statement(s).
- Auto Insurance/papers.
- Rental Lease/Receipt/Mortgage.
- etc.
