Study Permit Refusal - Reapplying offline, is IMM5957E required?

Hi, I applied for study permit on 15th June 2018 through online application for a univeristy but it was refused on below grounds:

1)Purpose of Visit
2)Personal assets and financial status

I will be reapplying in few days through paper based application now with all the required documents. I will be reapplying from India and I can see that there are two forms - IMM5818E and IMM5957E(University Students).

Now my question is in form IMM5957E(University Students) there is a below declaration which states that I need to declare that "I have never been refused an application submitted for a Canadian study permit" but since I have been already refused for study permit form IMM5957E seems unnecessary but it appears to be part of application package. So I am not sure if I need to include this form in my paper based application or not.

submitted for a Canadian study permit. Misrepresentation of any material fact will lead to my being
refused and being found inadmissible to Canada for a period of 5 years. I agree to the Canadian partner
institution providing information to the visa office regarding my enrolment standing at request of the
visa office.

Please advise if I can go ahead with only IMM5818E and if I can avoid IMM5957E, any valuable inputs by fellow members whose visa was refused earlier and who chose paper based application route later. By the way just to give some more insight on my online refusal I didn't include SOP as it was optional, I'll be including it now.

Please provide any valuable inputs if possible.

My visa has not been refused but is delayed, maybe due to lack of toefl validity. So its better to submit all supportive documents !! Sop, cv, letters , financial, etc best wishes!!

Hi, thanks for the response. I'll include all the documents. My question is with regards to inclusion of form IMM5957E as it is a part of application package and as per this form there is a declaration section which states that I need to declare that my Visa had not been refused before.

But since my visa is already refused before I can't sign this declaration. So want to confirm if I can exclude this form from my application. Hopefully I get a clarification from someone.
