I have a stupid question. On the sponsor contact information section where it asks for your adress, it says street number and street name. If my adress is 555 Smith Street, isn't 555 my HOUSE number? Or is 555 my street number?

Also what is the district?
 
No, 555 isn't necessarily your house number, e.g. 2030 East Street, (fictitious address) could be a block of apartment buildings or townhouses and the "house" number is 12A (apartment/townhouse 12, block A). Just fill it in in the way that is most logical, try not to read into it so much.

Some towns are divided into "districts". If this is not relevant to you, then don't put anything there.

your house # is the number attached to ur house for example 880 riverdale rd 880 is my house number riverdale is name of street district is applicable if it applies if not leave it blank
 
Take the address of the forum owner for example:

Campbell Cohen Law Firm Inc.

215 Redfern Avenue, Suite 118
Montreal, QC, Canada, H3Z 3L5

"118" is the house number; "215" is the street number; "Redfern Avenue" is the street name
