still need SOP, background declaration, etc for a study permit?

I've found some information on Google that students should submit the followings,

#1. Statement of purpose
#2. Personal history form (every details after 18y) (or background declaration?)
#3. Proof of what I've been doing past 5 years
#4. Transcripts of secondary grades and any post-secondary transcripts

The problem is that none of the information above are not required when I submit an application online on CIC website.
I also couldn't find any forms for them. All I can see is that there are some boxes that I could write down the information above on Application for a Study Permit Made Outside of Canada [IMM 1294]. But it would not be enough space to put details and not be in separate documents.

Are those requirements above (#1-#4) out of date?
Or do I still have to submit them?.

#3. Proof of what I've been doing past 5 years- this is part of the form.
#4. Transcripts of secondary grades and any post-secondary transcripts- this is part of the the school requirements.

sometimes, it's confusing too many information...
