Hey all,
I was on H-1B which expired July 2013 in the US and I left for Canada August 2013. In order to maintain status between the gap, the lawyer had filed a B-2 VISA (I think right before expiration of H-1). However, by the time they processed my B-2 VISA, it was voided because I already left the States and was living in Canada on a work permit.

My question is does this count as yes in the question, "Has (name) ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country?"

It doesn't count as a yes. If they had refused the B2 visa, instead of voiding it because you had left, it would. You were not refused the visa - in fact, it seems it was granted - but then it was rescinded because you no longer needed it.
