Hi, I needs help. Could someone tell me when to use this form (IMM 5409). If you are married and a son but they are not accompaneing you to canada. Is this form needs to be filled-up or no? Are those all forms need to be filled up and should be submitted to CIC?

> Instruction Guide [IMM 5487]
> Application for Work Permit [IMM 1295] PDF, 222 KB
> Use of a Representative [IMM 5476] PDF, 55 KB
> Statutory Declaration of Common-law Union [IMM 5409] PDF, 97 KB
> Document Checklist [IMM 5488] PDF, 30 KB

Thank you for your help,

If you are married, you don't have to accomplish Imm5409. Also check the country specific "application kit" of the region you are applying from.
