Hello,

My common-law partner and I will submit our Inland application soon. The form IMM5409 is not on the checklist, but I wasn't sure if I should still submit it. I think I saw a post somewhere that some offices requested it later even though it was not required.
Please help!

It is an optional form, although CIC has been known to ask for it later in the process for some couples; no idea what their criteria is for selecting those that need to submit it.

If you're thinking about including it, you might as well...just in case.

We are finally ready to send in our forms but Just went over the paper work and have a few questions of concern and confusion.

Form IMM5481E (Sponsorship Evalution) Number 4- I do not understand the Undertaking it says yourself , current undertakings, previous undertakings, and Persons not included. I am not sure what to put I dont have any previous or current undertakings but it sys not yet in effect also so am I going to be in an undertaking if its just my spouse coming here?

Form IMM5406E (additional Family Info) It says the principal applicant (my spouse) and their spouse (Me) must complete their own copy. Does this mean I need to complete a copy to or is this for only the the applicant (my spouse).

Form IMM5669E (Schedule A Background/Declaration) Number 8 personal history it asks for dates from & to, what do we put if its too present date and still working.

Thanks again for all the help!
