My story goes like this:
I am a Serbian citizen 28 years old. I'm working on a cruise ship in a restaurant, where I met a girl from Canada (27 y.o), and how it happened to be, the love of my life. She was not a a guest on the ship, she is crew, working in retail. We never "blamed" the ship that we got together but it helped a lot. For a short period of time our relationship grew exponentially. We were together all the time. On the ship we made plans to spend the vacation together, that she is gonna come visit me. At one point everything felt right. She was the one. No doubt about it. I proposed. She said yes. We didn't want to get married until we met our families. Our contracts ended and we went for vacation. Our vacation was two months from which she would spend the first month in Canada with her family, and then come to Serbia. In that month (May) I worked on my Canadian tourist visa to go visit her after our new contract that would end in January 2015. On May 30th they granted the visa for me which is from May 30th 2014 to March 1st 2015. We would do a civil marriage then in Canada and start working on my PR visa. I googled a lot about this whole thing and it is overwhelming for me and I can just imagine how it is for a Canadian to cope and understand how much effort there is to do any kind of visa for that matter. She already "met" with those feelings and obstacles when she arranged the papers for my tourist visa btw her brother is the one who sent me an invitation letter. Just to mention wherever I go for advice everybody asks me or just looks at me strange like I am doing this out of interest. Believe me when I say that I want to be with this girl for the rest of my life. But lets face it, Serbia is not that good of a country career-vise, ship is not a good option when you want to build a family.
Due to my job on the ship, and not being able to surf all the time cuz the ship charges a lot for internet and is really slow, I would really appreciate YOUR help
Basically I will start collecting all the stuff now until January but I need some more information and advices.

1. If I'm not mistaking I have to mail everything in there is not an online option. I ask about this because when I worked on my tourist visa the online option goes much faster than the mailed in one.

2. Since we are arriving lets say on January 15th. I have 45 days to apply (since my visa ends on March 1st).
- Should I then apply for a visa extension together with the PR application or earlier?
- If I apply for the extension should I include a copy of our marriage license as proof that I'm also applying for a PR visa?
- What are the chances that they deny me the visa extension? If I would have to leave the country my whole application is gone, I lose money and I have to start all over again, outland.
- How do I apply for a working permit? Do I apply together with the visa extension, or does (like in Germany) the working permit act like a visa, so I can basically stay and work? What if I found an employer that wants to hire me? How can he help me out with getting the working permit easier?
- I see that it currently takes 55 days for the 1st stage of processing for my PR visa. That is the evaluation of my wife to be a sponsor. I read that after the 1st stage, if I apply for the open working permit, they will grant it to me. Then they start the 2nd stage, evaluating me and our relationship. Do I have good information and understanding about that?

3.1. Are there any tricky papers that she needs to get asap or that there is a long waiting time so that we are on the ball with that?

3.2. Are there any additional paper that she can add to the application that could help us out.

4. Do I need to do a medical asap when I arrive in Canada?

She is now next to me, in my home in Serbia, we're in bed and she is looking at the things I'm writing. My head is bursting with information but at the time I think that is all I can think of.

If anybody has any tips please let me know.

First things first is to decide if you are applying inland or outland,With Outland you can still stay in Canada and apply for an extension of you visitors visa.
Once this decision is made then you can think about medicals, back ground checks etc. Either way you can not work until the first stage of processing inland which is about 10 months or when you get your PR from applying outland.
 
We plan on doing it inland cuz if nothing she can find a job and we would live from our savings from the ship. How risky is it compared to outland. Can CIC deny me a visa extension if i prove that i got married or will it be a plus. Doing it outland means that for the same amount of time we will be doing nothing and we don't want to go back on the ship + we want to be together. Thats why i think it is better to do it inland?

If you apply inland, you can't get an open work permit until stage 1 is finished. For inland that is 10 months according to current times. However inland does allow you to stay in Canada during the entire process which can take up to 18 months. During this time, you are not allowed to leave Canada, for any reason.

If you apply outland there is no open work permit. Serbian citizens are processed at Vienna office, and the whole process would probably take around 1 year. During this time you could only be in Canada with a valid visitor visa. If you can't maintain your visa, you would need to leave Canada and wait for your PR in Serbia or some other country.

To answer your questions.
1 - Everything is mailed. No online option.
2 - You could have everything done in 45 days. You would need your marriage certificate, police certificates from every country you've lived in for 6 months or longer since age 18, and your medical exam done. Often the police certificates take the longest to get.
As i mentioned, inland stage 1 is 10 months. Expect not to work at all in Canada until then.

You will need to also include proofs/evidence that your relationship is real. So photos, joint finances, letters from friends/family, etc
4 - Medical should be done as the last step before you send application, since it expires in 1 year.
