Don't put anything for employment or employer, select Open Work Permit as the type. For OWP fees: 100 + 155 OWP holder fee.

When you send, you may want to use a courier to make sure they have to sign for it.
 
Also, a good idea would be to include a copy of your upfront medical (if you have done it) with your OWP package. Otherwise they could place restrictions on your OWP to protect the vulnerable (kids, etc). Whether they place this restriction is all dependant on your country of origin. Someone usually says this on every thread, so let me ask, are you from a visa-exempt country? For most countries outland is a lot faster than inland. If visa-exempt, renewing your status shouldn't be a problem.

I am from Poland, and we are prepared for the long time of process - just were confused about the implied status. :)
