Sponsorship Paperwork being returned for 'Organization and Entities'

Hi everyone ,
I just spoke with a nice lady at the IRCC who (after apologizing) told me our paperwork is being returned.
It was received August 12.
Reason for returning to us:
Organization and Entities
Plus it was mailed back to us September 25 to Ottawa from Mississauga and we haven't received it yet.

There is a question on the Schedule A:

Have you ever been a member of an organization, such as a trade union, professional association, or political, social, youth, or student organization?

How did you answer that question? If you said "Yes" did you provide specifics of which organization or entity?

Yes, I did answer that. I'll take a look again. Hoping it comes back in a timely manner.
