When you send in the sponsorship and application, should you also send in medical and police check or wait til asked to do so? We are in Canada but applying outland.

which visa office are you using?
 
Family class applications MUST be accompanied by the proof of medical exam, and police certificates unless specified otherwise in your Regional Guide (some police authorities will not issue the certificate without proof of a request from the embassy). Double check the guide.
 
He is from St. Vincent, so POS visa office
 
So he should get a medical here in Canada and send it in with his application and also send for background check
 
Yes, he will need his medical exam done at at designated medical practitioner, see here: http://www.cic.gc.ca/dmp-md/medicalinfo.aspx?CountryID=2069&CountryName=St.%20Vincent%20and%20the%20Grenadines That's the only DMP available in that territory.

He will need his PC for St. Vincent and anywhere else he's lived for 6 months or more. See here: http://www.cic.gc.ca/english/information/security/police-cert/central-south-amer/st-vincent.asp

Both the PC and the proof of the medical exam (the IMM1017 copy will be given to him by the DMP) should be included with his application.
