Sponsoring my wife

I am preparing application to sponsor my wife. But, i am confused with some parts. I need your help, anyone.

1. Do i need to include Statutory Declaration of common law union(IMM 5409) and Use of representative(IMM 5476) with my application even if i don't have anything to fill up those?
2. Do i need to write NA in every field of the question which does apply to me even if i chose NO for that question?
3. For my wife's application, does she need to include me in her dependent declaration form?

Any help will be much appreciated.

Thanks tons for the response. Pretty clear cut noe. So continuing with question 3, if no children can she skip additional dependants form?
