I live in Ontario and my girlfriend is from NY state. She has been living (visiting) with me since Jan 1, 2009. She goes home a week at time every month or two. We both go visit her home probably once a month. She has been allowed entry every time she comes back. She still currently has an address and job in NY. What do I put down for Q27? Have you lived together, give dates.
Also what do I put down for Q28 are you currently living together?

I'm asking these questions because legally she is only visiting me here in Ontario. She has an address in NY.

Are you applying common law? If so, you have to prove you have been living together for a year, so will have to use your Canadian address as her address.
If you are going to get married, and then apply, I would put her American address as her address, in particular because she still has a place and a job in NY.

Then for the other questions, state you are living together, but attach a letter of explanation. Lots of people on this forum have been technically 'visiting' in Canada, but were really living here, and have put the Canadian address as their residential address, and have had no problems with the application.
For Q27, I would say 'yes', and give the dates, including her time 'visiting' you in Canada.
For Q28, I would say 'yes'. Again, attach a letter of explanation, and then even if the visa officer does not consider that to be 'living together', he or she will at least see that you were not trying to cover anything up.
