SOLVED - Is that passport request?? (Manila)

Hi, my wife (from Japan) received an email from Manilla office asking her to fill a form called Appendix A (joined with CIC email), and at the bottom of the form it is written Passport Request.

(I uploaded the image of the Appendix A)

Is this the passport request step? Do she has to send her passport? We didn't see any specific instruction to send the passport to the Manila office.

Japanese do not have to submit their passports. Follow the instructions given in the email.
