Small road Block on IMM 5476 any help would be appreciated. Thank you

Hello,

I am almost done with the file. I have also read the instructions however was little concerned if I have to fill IMM 5476 Form ( Use of Representative). In checklist it doesnt say "optional or if applicable in front of it".

Is it Mandatory?.
If yes then do i have to just fill my name, Client Id and Signatures? .

Please suggest me that will be a great help

If you are using any representative (imm lawyer, consultant etc..) then you have to fill that form...

If you are not using any representative and applying on your own then you don't need that form in you package... just don't send that form and don't put a check mark in the checklist

If you don't include optional forms in your application, mark them as 'N/A' on the checklist. That way the VO
knows you've acknowledged the document and chose not to include it.

You mean by hand, after printing the forms? cuz we cannot write anything in the checklist...

Correct. I also checked all the checkboxes by hand, one a time as I completed forms and included documents.

i did not put anything for the documents i did not attach and i passed the first stage too.

Ya, I'm not saying you have to follow and do what I say, I was just saying that if you don't put
anything on the checklist and don't attach the form, it might look like you've forgot to include it -
whereas if you mark the form as "n/a" on the checklist, it's clear that this form is not included on purpose.
