Simple questions regarding family class sponsorship

Hi all,

I'm on Post Graduation Work Permit until August
My wife is sponsoring me and I got couple questions:
I will send my application for family class this month by mail. Should I send it with extension of my open work permit or I it has to be done an online?
I want to send my medical test upfront how can I do that?
