My wife got her documents translated and notarized from Chinese to English but the forms they returned to her had an additional page at the front that says 'Notarial Certificate' for the title.

I'm wondering if this page needs to be translated too or should be removed?

Shouldn't this page would be translated if you bring the whole document to the local notary public (which includes translation services) in Canada?

I am confused with your question. Isn't this the Notarial Certificate? Did they also translate where this notarial certificate is being issued? (the bottom line)
 
So this page wasn't part of the documents my wife sent to get translated and notarized.

For example, if my wife sent a copy of her birth certificate which is 1 page to get translate and notarized, they would send back 4 pages total. The title page that I showed in my first post, the untranslated birth certificate as the second page, the translation for the birth certificate as the third page and finally a fourth page with all the information in English about the notary/ translation company that was responsible for this translation job.

Now I'm wondering if we should include that first page when uploading my wife's documents for this application or not.
