In my mark sheets and transcripts... I have Tomkumar Shah.....and at some place only Tom Shah. So it's ok if I will mention this in column3(other names you are or have been using). or I don't need to write this....pls advise.....

Yes write this in other names otherwise they might question you on your transcripts.

Write Kumar in other names bracket Question 3 dont have to attach a seperate sheet.
