Section A for spouse is missing in medical request

Hi,

I got a medical request from CPP- Ottawa. I applied under CEC 25-AUG-11 and I got the request 19-OCT-2011.

They have sent the section A form for me , but missing sectoin A for my wife.

Is any body who recetly get the request from CPP- Ottwa having the same problem ?

Kindly reply on the same. I want to complete me medical exams before this month end as I am going back to my home country.

Duke, contact your visa office and ask. You should have a form for your wife too.

I have asked visa office. Can I alone go ahead and do the medical test and later when they send the medical forms for my wife , she will do at then.
