I am planning to submit schedule A along with my eAPR. I have more entries than the rows that are on the form. How can I fill the form correctly. Also shall I upload it in the letter of explanation placeholder?

create a covering letter for LOE section and mention that you are also adding schedule A because of lack of enough rows. Merge schedule A with other LOEs

You need to add a separate sheet which you must type it out for "8. Personal History" to complete this section of the form.
If you read the first few top lines of Schedule A form, they have mentioned to use separate sheet to complete the section where you run out of space.
Also, make sure to write your name in your native language after printing the form.

You don't have to sign the LOE.

Yes, first page LOE or Index followed with the documents and form. Index is not mandatory, however if you add one, it will look good anyway.

in my request of schedule A today, it is mentioned date format (YYYY/MM/dd). but in the form, it is given YYYY-MM. should i include dates or month and year is fine?
