Schedule A Request

My PR application is being processed and recently I received an email from CIC telling me to submit a

"Schedule A: A newly completed Schedule A with Question 8 and details as required."

Does this mean that I still need to fill out the form entirely or just question 8? Also, why might I have been sent this and how do I make sure that I do it properly?

Just to be clear, I didn't answer yes to any but I do have a very long address history.

Also, do I need to indicate my ~2 week travels as different activities? It's not like I stopped working to go on this travel.
