Schedule A Background declaration

hi everyone good afternoon... help pls

i already validated the form schedule a background declaration but there s no barcode that appear. is this form have a barcode or not? thanks

After being validated, only IMM1344 and IMM0008 forms will create barcode pages.......IMM5669 (Schedule A) form will not produce a barcode page after it’s validated.

i have 1 more question
is it okay to print the forms without putting the date ill just write it after signing? or put the date print then sign?
