I am sponsoring my mother under the family class, we are completing the Schedule A Background/Declaration form. It is requesting information on her parents birthdates and date they were deceased. No one remembers these dates. Has anyone had this problem and how did you handle it?

In IMM5669 - Schedule A form, the date of birth field is a mandatory but date of death (if applicable) is not a mandatory field.

Since you must provide your Parents Date of Birth which you don't know........write 1900-01-01 and provide explain on a separate sheet.

Leave the Parents Date of Death BLANK.........provide an explanation on a separate sheet.

Guide 5772......Schedule A – Background/Declaration (IMM 5669).....Question 3.......date of death (if applicable).

Note: when you asked to upload IMM5669 - Schedule A form in your Online Account........you may not be able to attach the separate sheet where you provided the explanation for your Parents Date of Birth and Date of Death to the Schedule A form.........and may have to upload only the Schedule A form without the sheet.............i think you should be okay as IRCC will review your complete sponsorship application package which includes the separate sheet you earlier submitted where you explained your Parents Date of Birth and Date of Death.

Could you assist with the Additional Family Information (IMM 5406) form? They are asking for information on all dependent children 18 years of age or older. My brothers are both over the age of 18 and are Canadian citizens and are not dependent on my mother (the principal applicant). Do we need to include them on this form?

Canadian citizens are not eligible to be sponsored and cannot be issued a visa.........your brothers who are Canadian cannot be added to the sponsorship application..........however, they should only be included in SECTION B of IMM5406.
