Schedule A 6d and 6e

Hello, I was sent an ADR (Schedule A) but I'm having a problem filling the 6d and 6e questions. I was denied students and visitor Visa in the past in Canada, so my 6d is automatically YES.
However, I haven't been denied entry or asked to leave any country, but I'm confused, does a visa denial mean denied entry??
