Schedule 1 Background/Declaration-Point 9

Hellow all!

I got temporary resident VISA to canada in 2008.
After that my first Application for Permanent Resident was refused as I was unable to submit IELTS results on time in 2009.
When I am going to re-apply how can I fill the "Point 9-bullet 2 & 3 of "the Schedule 1 Background/Declaration form"?

I think you should say "yes" and then state the reason below or in an additional sheet of paper. I have read a case in this forum where the applicant had lied about being rejected earlier (his lawyer asked him to do so) and then rejected later during interview because he hid the fact.
You could say that you were unable to provide sufficient documentation before and state reasons. Thats what i think
