RPRF with full docs

i m planning to send RPRF with full documents to CHC, is there any fee payment form to be filled for RPRF as we do when applying to cio.

No form, attach a forwarding letter mentioning file number, DOB, draft No and other particulars. Write your name, file number and DOB on the back of draft

Yeah should be fine. Sent mine with the full documents, too, and got a receipt for it.
