Request for Option C for 2010-2012 years but not available online/phone HELPP

Hi

I finally received some correspondence and need to send in the option c print out - but I am unable to retrieve it online or on the phone. What have other done in this case? I only have access from the ones in 2013-2015.

They should be able to mail out.
Call CRA I don't see why you cant get it.
