I think that's an old one. Saw that on google images.
I called up service ontario and they said I had the correct one?
Long form, just a bit longer than A4. Top says "Certified A True Photostatic Print of a Record" and has a certificate number.
Then in the space below is a photocopy of our Marriage licence with all Parts (1,2 and 3) filled in and signed at the bottom by the deputy register with a Register General stamp.

I think I read somewhere that there are applicants who had to send in the short form Ontario marriage certificate as per request frm cic.
I am still waiting for mine to be registered, and I think I'll submit both just to be safe. Lol.

oh really??? cause that one looks exactly like the one I sent! Oops... I didn't send the long one.

There has been so much confusion with reg. to the Ontario Marriage certificate. I read posts where people only sent the Record of solemnization. And some sent only the long form. I sent both. I did not include the short form but I have ordered one though.

There is no consistency in what they want. We have to take our chances.

I would suggest you send a color photocopy of all 3 this time. Good luck !
