Renew PR and then leave

Hi,
I and my spouse are Canadian PR holders. We did our soft landing on 21/May/2016 and left 2/Jun/2016 (12 days).
We finally landed in Canada on 22nd April 2019 and will complete our two years on 22nd April 2021. During this time, we spend one day aboard. My wife would like to leave for the USA ASAP to give her USMLE 3 exam and pursue a few medical internships there.
Can she apply for her PR renewal at the end of April 2021 and then leave Canada? I will receive her PR Card. I can travel to USA to give it to her.

You can apply to renew your PR cards up to 9 months before they expire. Yes, she can leave Canada.

If doing a residency in the US continuing to meet RO requirements will likely be an issue unless you remain in Canada and can sponsor her again.

Given that she will be just over the 730 days she may require a secondary review. In cases where someone is so close to their RO you can be required to pick up the PR card in person.

Yes, most likely she will not be able to meet RO for Canadian citizenship if she gets a US medical residency. I just want to renew her PR so that she can travel freely between US and Canada.
I will remain in Canada until I get my citizenship

She, not only will not be able to meet RO for Canadian citizneship, she will not be able to meet RO to remain a PR.
In case she gets reported in one of her trips and her PR get revoked, you can apply to sponsor her again.

As I mentioned before if she is just over the 730 days there is usually extra processing time and the person often has to pick up their PR card in person.
