Relative in Canada

My sister living in Canada (I have shown her as my relative in the form) got one letter from CIC on Thursday. There is her name in the envelope and the form is blank. There is no name or number or anything relating that it is for my case. It says to fill the applicants identification with her. The problem is, there is another applicant too who has shown her as his relative.
Now we are confused, whom the form is related to. There is a toll free number on the form in which she is going to call on Tuesday.
Lets see what happens. Anybody else got same type of form? please share.
I have not got 2nd AOR yet from Buffalo. Don't know how long it will take more...

PROOF OF RELATIONSHIP IN CANADA (IF APPLICABLE)
Submit:
•Proof of relationship showing you have, or your accompanying spouse / common-law partner has a close relative who lives in Canada and is a Canadian citizen or permanent resident. This close relative can be:
•parent, grandparent,
•child, grandchild,
•brother, sister,
•aunt, uncle, or
•niece, nephew.
•Proof of relationship to your close relative in Canada, such as birth, marriage or adoption certificates. For example, to prove that your relative in Canada is your paternal aunt, it would be necessary to submit copies of birth certificates for her and for your father showing they have at least one common parent.
•If your close relative is a permanent resident of Canada: provide a photocopy of his or her Record of Landing (IMM 1000), Confirmation of Permanent Residence or Permanent Resident Card.
•If your close relative is a Canadian citizen: provide proof of Canadian citizenship, such as a photocopy of pages of a Canadian passport or Canadian citizenship card.
•Provide evidence to show that your listed relative is currently residing in Canada. This may include copies of:
•lease agreements,
•a current Notice of Assessment from Revenue Canada,
•an employer's letter confirming employment,
•monthly bills etc.
Note: The evidence should show the relative's name and full address in Canada.

I don't know what you already sent but it looks to me that you need at least yours and your sisters birth certificates showing that you have the same parents, a copy of her PR card or Canadian passport and some proof that she is residing in Canada.

Yes, I sent my and my sister's birth certificates, her canadian passport, tax assesment paper and utility bill with the application. there was a form also we both signed stating that we are siblings from the same parent and she will help me whatever way she could when I will move.
Don't know why they send this one again now.

They may have lost it or they may have overseen it while flipping through your application papers. I hope you made copies so you can send it again.

In such a case, aren't they suppose to contact me? they send this to my sister in Toronto. So I was worried. Should I send an email to them?

Well, maybe wait a bit and let her call to find out. Maybe it is the other applicant who was missing some documents.
